EXCELSIOR (Impact of Extent of Clopidogrel-Induced Platelet Inhibition During Elective Stent Implantation on Clinical Event Rate) trial reported by Hochholzer et al:

• Genotyping of CYP2c19*2 carrier status alone explained only 5.2% of high residual platelet aggregation

• When the CYP2C19 genotype was added to clinical characteristics, only a mere 11.5% of poor clopidogrel responsiveness was accounted for

• The sensitivity of CYP2C19*2 carrier status in detecting patients who have high on-clopidogrel aggregation was only 45.1%

• The specificity of CYP2C19*2 carrier status in detecting high on-clopidogrel aggregation was only 75%

The authors conclude that “genotyping alone or in combination with clinical factors [cannot] replace phenotyping of platelet function.”

In another study presented at Euro PCR this week by Bonello et al, “one out of five patients who carried the now-infamous "poor-metabolizer" gene variant actually responded appropriately to an initial clopidogrel loading dose, while one out of two who were not carriers did not.”  Read the article here.
